Practitioner Home-Visit Program


The service provided by the Practitioner Home-Visit program is available for any resident in Assisting Livings or Retirement patients in their own residences or Communities. This is a very convenient way to avoid distressing your family or friends, and avoiding expensive taxi fees. Just call for an appointment and we will bring a provider to your own residence.

Our group of providers, all certified internists and family physicians, will be traveling to patients facilities who are interested in participating in this program. If you are a patient who wants to be seen at your own residence or facility, please let us know as soon as possible. We ask that you carefully read the details of this program and give us a call.

Policies:

  1. Patient will need to call 303-337-5575 to schedule an appointment. Our operator will inform you when the provider will be at your location.
  2. Emergency appointments are not available.
  3. If you are a self-pay patient, you will be asked to pay at the time of visit.
  4. You must have the requested health insurance information available for the operator. We accept all insurances, except Medicaid. If you have any other insurance, we must obtain a prior authorization for a home visit. You must have your insurance card with you on the day of the visit.
    Your insurance will be billed as follows:
    • New Patient: depending on the complexity, we will bill your insurance $250. We will contact your insurance to make sure that the home services are covered.
    • Established patients or Follow-up: Depending on the complexity of the illness, we will bill your insurance $200.
  5. The doctor will be at your residence within 2 hours of your scheduled appointment time.
  6. Your doctor will also bring equipment for acute care (diagnostic and treatment).
  7. Laboratory analysis is not available at this time, except for some basic laboratory tests.
  8. Flu shots and other immunizations are available
  9. We will keep records of the procedures and evaluations performed during the visit in accordance with the HIPAA compliance plan. We will also keep your assisting living nurse informed of any changes in your current health conditions and treatments.
