Become A Patient

Rocky Mountain Internal Medicine is continuously accepting new patients. We accept all insurances except on Medicaid, but even then exceptions are made under special circumstances.

In order to become a patient, first call and schedule a "New Patient" appointment for yourself. Then either simply walk in and fill out the required paperwork, or to save yourself time, simply download it below, fill it out, and bring it in.
